Prior to having an evaluation with one of our therapists, a doctor's order is needed. The doctors order can be faxed to our office at 815-462-4929 or you can bring it with you on the day of your evaluation. It is suggested you check with your insurance company to see if the service you need is covered by your health plan. On the day of the evlauation, we will also need a copy of your insurance card along with the following forms:
